Annual Meeting December 7th to Highlight Critical Time for WCHN Providers.

It’s urgent. Our healthcare system is rapidly transforming from a care delivery, fee-for-service model where we are compensated individually based on volume, to one where we will be collectively reimbursed based on performance outcomes and value. And the transformation is accelerating. Recognizing this reality, Western Connecticut Health Network’s PHO/ACO has been creating a clinically integrated network to meet these new demands and to meet the tenets of the Triple Aim:

• improving the health of a population,
• delivering better care
• lowering overall medical costs

Right now our PHO is developing new value based contracts which share risks and rewards among providers, insurers, employers and even patients.  Through WCHN, the PHO has partnered with the Value Care Alliance (VCA) and its four hospitals to make us additionally valuable to insurers and patients by offering a larger network of providers committed to delivering high quality lower cost care.  These are strong reasons for you to consider joining the WCHN PHO.

WCHN PHO invites all providers to attend its 4th Annual Meeting in the Robilotti Conference room at Danbury Hospital on Monday, December 7th, 2015 at 5:30 PM.

This year’s annual meeting will highlight the efforts underway by the PHO and its physician leaders to prepare the organization for clinical integration and accountable care.

The meeting will feature Dr. Tom Graf, a nationally recognized leader in developing comprehensive programs that address patient care across the continuum. Dr.Graf was the Chief Medical Officer of Population Health and Longitudinal Care Service Line at Geisinger Health System and most recently joined Chartis Group as National Director of Population Health Management.

The meeting will also present an update on the PHO’s activity for 2015 and will hold an election of the Board of Directors.

WCNH PHO has over 881 providers. We are a strong and vibrant organization committed to continually improving the programs and services that we provide our physician partners. We want to thank and congratulate all of you who have made this journey possible.
