American Hospital Association Recognizes Community Care Team

The American Hospital Association's Hospitals in Pursuit of Excellence team will be recognizing the success of Norwalk Hospital's Community Care Team (CCT) in their upcoming guide highlighting case studies using Triple Aim strategies to improve behavioral health. The guide will spotlight programs at select hospitals and demonstrate how they partner with community stakeholders to improve behavioral health. Norwalk Hospital and the CCT is one of only five hospitals that have been selected to be showcased.
